Care Quality Commision have awarded the surgery GOOD to OUTSTANDING ratings across all domains assessed during a recent routine on site inspection. You can read the full report here.

To increase our capacity to deal with the demand for same day appointments we are pleased to announce that from August the Drop In Clinic will run Monday to Friday -including Thursdays and will start at 8:30am-10am. There will also be Minor Ailment Clinics in the morning/afternoon across the week.

The surgery will be closed on Thursday 9th and Thursday 30th July from 1pm for training purposes -after this time please call 111 to access the GP Out of Hours Service.

We are aware that the exisiting telephone system is not satisfactory and we are looking to replace this with a new improved system in the near future -we thank you for your patience during this time.

All Patients are being allocated a named GP, this will be the doctor you are currently registered with. You can request to see any GP -not just your named GP.

Please do not email a request for a call back or put a call back request on the online feedback forms - the emails/feedback forms are not checked on a daily basis and we will not be able to process this request.

Please nominate a chemist to receive your prescriptions electronically using our new electronic prescription service (EPS) - this can be done at reception or the chemist.

New drop in clinic days: From the 19th January 2015 the morning 8-10am drop in clinic will run every weekday morning EXCEPT THURSDAYS and be staffed by our new Nurse Practitioner Elizabeth Williams. Elizabeth is qualified to diagnose and prescribe medication and will also be available for same day telephone advice and assessment in the afternoons.  This will free up a lot more routine GP appointments across the week.

We ask that you no longer rely on the surgery sending out invitation letters reminding patients to attend for their annual chronic disease nurse reviews. Please take responsibility for arranging your own chronic disease annual review. To spread these reviews evenly across the year (and to make it easy for you to remember) Please ring to arrange to be seen for this IN THE MONTH OF YOUR BIRTHDAY ..... even if you have had your check up in the last 12months.

Our patient satisfaction survey and annual report are now available on the website.

All patients are advised to please read about the NHS CARE DATA PROGRAMME or SUMMARY CARE RECORD in the further information section. This national programme has requested some of your information is shared to improve healthcare. You have a choice to opt out of this. Please read carefully and contact the surgery reception staff for further information or to opt out.

Appointments

All GP surgeries are extremely busy at present. We are constantly trying to provide as many appointments as possible and appreciate your patience when ringing to book an appointment. We are delighted to inform you that Dr Lucy Hyland (GP Trainee ST1) has joined us from February and will be with the surgery for 12 months. This will result in a greater number of appointments being available. 

Drop in clinic

For URGENT problems or if you are UNWELL and feel you cannot wait until the next routine appointment is available with a GP we offer a drop in clinic between 08:00-10:00AM with the Nurse Practitioner who is qualified to diagnose and prescribe. The Drop in clinic is NOT AVAILABLE ON A THURSDAY.

Please be aware this is a shorter than normal appointment and only ONE problem can be discussed. There may be a wait and patients are seen in arrival time order. 

Clinician advice

We also offer telephone consultation appointments with the GP of your choice as well as same day nurse practitioner call back for advice and guidance for urgent problems.

Training Surgery

Dr Owens has qualified as a GP trainer and the surgery has been approved as a training practice. They are medically qualified and GMC registered doctors who are able to treat patients independently but also have a more experienced GP available at all times from whom they can seek advice.

Dr Lucy Hyland will be training at the surgery until February 2016.

In addition to this we will also continue our partnership with the University of Liverpool and provide education for medical students at various stages of their training. Medical students are not yet qualified doctors and all patients will be seen immediately after the students by a qualified GP.

Greater Online Access to your medical records

From April 2016 we intend to give patients increased access to their medical records online......further details to follow.

Flu Vaccination

Please BOOK IN AT RECEPTION NOW for your Flu Vac appointment. Protect yourself EARLY before its too late! Anyone who is 65years of age and over, pregnant, a carer or who has certain medical conditions is eligible to have a Flu Vac on the NHS. If your not sure if you are eligible please ask at reception.

Additional appointments evening and weekends....

This practice is now able to offer patients the additional choice of evening or weekend non-urgent appointments. These appointments are available to patients across Warrington as part of a Warrington Health Plus initiative...... Making it easier to see a GP in evenings and weekends.

The appointments are available at Bath Street Health and Wellbeing Centre between the hours of 6pm to 8pm weekdays and 8am to 8pm on Saturdays and Sundays, for non-urgent patients only, on a first come first serve basis. The appointments must be booked in advance and can only be accessed following referral to the service by your own practice. It is not a same-day appointment service. If you would find a later or weekend appointment more convenient, please ask the receptionist when booking a non urgent GP appointment

We continue to look for ways to improve access by offering the morning drop in clinic, telephone advice and opening late until 8pm every Wednesday -this is in addition to running routine clinics between 8am and 6:30pm. We ask that patients also take some responsibility and support your GP Surgery. Ways that you can do this includ: Cancelling unwanted appointments ---Seeking appointments/advice when appropriate - pharmacys offer advice and over the counter preparations --- Using our self help guide ---Only requesting Home Visits for truely housebound patients.

New Driving Law March 2015

A new law on driving after taking certain drugs is coming into force in March 2015. Please continue to take your medications as prescribed by your Doctor and see the following link to a patient leaflet for more information.

Carers

Are you a carer? Please inform the surgery by completing our Carers Form. You may be entitled to benefits, support and the flu vaccination

Summary Care Records

We now offer patients access to their summary care record. Ask at reception to register for online services.

 

Chronic Disease Reviews

Patients are now asked, as far as is reasonably possible, to attend for their annual chronic disease nurse reviews eg diabetes, blood pressure, heart & lung conditions, mental health and rheumatoid physical health check ups in their birthday month. Morning Drop in Clinic & Minor Ailments Clinic

We continue to operate the morning drop in clinic from 8:30am - 10am Monday to Friday. For other urgent matters after 10:00, the receptionist will ask you for appropriate medical information and contact telephone number and add you to a triage list. The Nurse Practitioner will telephone back with advice or ask you to come into the surgery for a consultation if necessary. There is also now a Nurse Practitioner led minor ailments clinic - Emma & Elizabeth are able to diagnose and prescribe medications for a wide range of conditions such as musculoskeletal problems, infections and rashs. If your not sure ask the receptionist staff who have a list of appropriate conditions. If the nurse is unable to help she can discuss with one of the doctors or arrange an appropriate appointment with the doctor.

Suffering with Low mood, panic or anxiety?

Psychological Services (iapt) now offer a drop in clinic on Mondays from 1 - 4PM at Making Space on Allen Street - WA2 7JB. You can also self refer to the service by calling 01925 401720.

Greenbank Surgery

We are confident that our website will provide clear and concise help and give our patients the information they require in an easy and convenient format. It has been designed with the patient’s needs at the forefront of everything, from checking surgery times to letting us know what you think of us. 

We now have a monthly Newsletter and from 16/10/13 we will sending out appointment reminders, practice news and health promotion text messages direct to patients who have registered a mobile phone in their contact details.
